Multi-Source Feedback or 360 Degree (MSF/360) exercise should be undertaken at least once per medical revalidation 5-year cycle. MSF, once completed, should also form part of the discussion at your ongoing annual Appraisal. Generally, an MSF is deemed complete when all three components were undertaken: Self Assessment, Colleague Feedback and Patient Feedback. If you need to reopen your MSF/360 which has already been completed, please request it from your organisation's administrator. You will find their contact details under Help & Support on your Dashboard. Once you are done amending the reopened MSF/360 (i.e., by adding more replies for a particular type of feedback), you will need to notify the relevant person at your organisation that your MSF is ready to be re-analysed by clicking on the designated button in the Analysis and Report sectFew readers
Colleague Feedback
I need to undertake Colleague Feedback
The first step would be to ensure that you have opened an MSF exercise on your account. You can access your MSF from your Dashboard either in Overview or by clicking on the MSF/360 tile. You can also use the My MSF/360 shortcut from the drop-down list under My Dashboard in the top-left corner of the screen. You will either be taken to the current open MSF, or you will be able to start a new one by clicking on the + New MSF/360 button on your Dashboard. Doctors can use SARD's Colleague Feedback form to collect feedback from colleagues they work with in any capacity, such as nurses, administrators and so on. You just need to ensure you are answering to the best of your knowledge. Completing the form would be entirely non-compulsory, so you would not need to respond to this request if you do not wish to do so. This would particularly be the case if you have been nominated for feedback in error. As soon as you are done nominating colleagues for feedback for your MSF/360, you need to notify your organisation's administrator or your appraiser as per the instructions in the Colleague Feedback section on your MSF page. The QR code for GMC patient feedback questionnaire can be found under Online Patient Feedback Options in the Patient Feedback section on your open MSF page. At present, we cannot accept scans of GMC patient feedback questionnaires sent by email for processing. If patient feedback was collected on paper, and if the Instructions in the Patient Feedback section of your MSF page indicate our Canterbury address, the questionnaires must be sent to us by post. We recommend using a special/signed for delivery service.
