How do I view my previous Appraisals?

You can find the summary of your last Appraisal in My eDocuments on your Dashboard. In order to see the full list, please click the View My eDocuments button on the right. How do I set/change my appraiser?

This would be added in Section 3 (Personal Details) of your Appraisal. The option to add a second appraiser will show in the final line of this section, with a tick box to confirm whether they would be needed. Once this has been selected, you can enter the name and email address of your second appraiser. My Portfolio is blank or missing sections
The Portfolio sections (datasets) are created for each user based on the medical roles they have selected for their account. Medical roles can be managed by the user as well as the organisation's administrator. You can set your medical roles on the Welcome Page (i.e. when you log in to your new account for the first time), and can edit them in the edit details page afterwards. Your Appraisal month is the same month each year. You should complete your Appraisal within your assigned Appraisal month each year to maintain Appraisal compliance. If you were to complete your Appraisal before or after your Appraisal month, it would be counted as early or late respectively. This is possible to change on the site. You can still edit or update an Appraisal that has been submitted to your appraiser while this is in Submitted state. You can check the status to the top right of the Appraisal page. Appraiser Feedback
I can't find the Appraiser Feedback form
The Appraiser Feedback form (questionnaire) is distributed to appraisees automatically once the Appraisal is complete. If you have received an email asking you to complete this form, a link to this will be within the text of that email. If this method does not work for you, or if you cannot locate the link, there would be an alternate method to find said form. The Appraiser Feedback questionnaire is automatically sent to an appraisee as soon as their Appraisal is deemed complete.
