Where can I find Appraisee Declaration Form?

Some organisations require a document called Appraisee Declaration (formerly Lead Clinician) Form to be submitted prior to the Appraisal.

Said form needs to be completed and sent to one's Lead Clinician to be commented on and signed off. Once this is done, the form needs to be uploaded to the relevant section of the Portfolio (usually Section 8 - Additional Information or similar).

If the link to Appraisee Declaration Form is not working or missing in the Portfolio, it should be available under Help & Support (Organisation Support Docs or similar) on your Dashboard.
