Where can I find a Declaration of Interest?
You may be able to complete a Declaration of Interest (DOI, also referred as ‘No conflict of interest’ form) through Section 6 (Private Practice) of your Job Plan.
At the top of the page there should be The DOI may be completed here link to an external site where you can fill in the form:
Please mind that the Declaration of Interest is an optional inclusion by your organisation, not by SARD. DOI's location can differ from organisation to organisation, i.e., it may be available under System Support (Job Planning or Organisation Support Docs) on your Dashboard:
If the link to the Declaration of Interest is neither in Section 6 of a Job Plan nor under System Support documents, please request it from the administrator at your organisation.
