# I can't find the Appraiser Feedback form

The Appraiser Feedback form (questionnaire) is distributed to appraisees automatically once the Appraisal is complete.

If you have received an email asking you to complete this form, a link to this will be within the text of that email. If this method does not work for you, or if you cannot locate the link, there would be an alternate method to find the said form.

Once logged in to SARD, click on the **Notifications** tab of your Dashboard. The notification relating to Appraiser Feedback would act as a link to the questionnaire itself. If you click on the notification ("An appraiser feedback form was sent to you on [date] and requires completing") itself, you will be taken directly to the form to complete.
