I am not sure if I can complete the Colleague Feedback questionnaire

Doctors can use SARD's Colleague Feedback form to collect feedback from colleagues they work with in any capacity, such as nurses, administrators and so on. You just need to ensure you are answering to the best of your knowledge.

Completing the form would be entirely non-compulsory, so you would not need to respond to this request if you do not wish to do so. This would particularly be the case if you have been nominated for feedback in error.

You would still be eligible to complete the CF questionnaire if you no longer work with the colleague in question, as this is addressed as part of the questionnaire.

If you have never had contact with the nominator in question, this would suggest you have been nominated in error. If yes, you may wish to contact the administrators for the organisation in question (or support@sardjv.co.uk, if the request has come from a separate organisation from your own).

For any questions you are unsure of, you can simply select the response for Don't Know, and your feedback will be included in their finalised report. This would be treated as a fully neutral response, and would not positively or negatively affect the MSF owner's overall report unfairly.
