Can I email scans of Patient Feedback forms to SARD?
At present, we cannot accept scans of Patient Feedback questionnaires sent by email for processing.
If Patient Feedback was collected on paper, and if the instructions in the Patient Feedback section of your MSF page indicate our Canterbury address, Patient Feedback questionnaires must be sent to us by post.

On average, it takes 7-9 working days for us to process Patient Feedback forms counting from the date of delivery to our office. You will be automatically notified once your Patient Feedback forms are received, and then upon processing them on the system.
Please ensure to include the cover letter in the batch of your Patient Feedback questionnaires, the template of which should be hyperlinked in the Patient Feedback section on your MSF page.
